TARDOC replaced TARMED for outpatient medical services in Switzerland. For a practice or a medical service, four points to keep in mind.
Billing is by the minute
A consultation starts with a position for the first five minutes, then each additional minute is added one at a time. No more five-minute blocks rounded up.
Every position has two components
Points for the medical component (PM) and points for the infrastructure and staff component (PIP). The two are added together, and the total is multiplied by the tax point value.
The tax point value is cantonal and annual
Under health insurance, between 0.82 and 0.94 francs depending on the canton, reviewed every year. Most cantons apply a provisional value that can be corrected afterwards; under accident and disability insurance the value is national.
Time without the patient is recognised
Reviewing a file received, information obtained from or given to a third party: each has its own position, by the minute, in the patient's absence. A report to a colleague is also billed by the minute, under a position of its own.
Outpatient flat rates cover standardised procedures. A primary care consultation is still billed per service, under TARDOC.
From consultation to invoice, without retyping.
Positions are attached to the consultation itself. Nothing to carry over at the end of the day, nothing to look up in a binder.
Duration first
The timer measures the consultation, you confirm the duration. The matching positions are set, and the link between time measured and time billed is kept.
Cumulation rules, checked as you type
Non-cumulable positions, a supplement without its main position, a cap per session: every deviation is flagged as you compose the service. Nothing blocks you, the decision remains yours.
Francs, not just tariff points
The amount appears in francs as you type, with your canton's tax point value applied, marked as an estimate until the invoice is issued.

Immutable invoice, QR-bill included
Validation freezes the invoice: sequential number, tax point value of the year of the service, exact amount to the centime. Archivable PDF with QR payment slip.
The tariff positions are loaded into your own instance: no call to an external service while you type. TARMED remains available for reference.

The same consultation comes to 62.98 francs with a tax point value of 0.82 and 72.19 francs at 0.94: the tax point value is set canton by canton, and most cantons set it provisionally for 2026. Twenty minutes is also the maximum that can be billed under these two positions, as additional minutes are capped at 15 per session.

The additional minute never stands alone, and stops at 15
It is always added to its main position. Beyond 20 minutes, only what matches a position of its own can be billed: organ system examination, explanation of a planned procedure, patient instruction.
Time billed is time actually spent
TARDOC counts by the minute rather than in five-minute blocks: a minute started counts as a full minute, and you bill the time spent.
What is done during the consultation is part of the consultation
Prescription, certificate, explanation to the patient: included in the time. The positions ‘in the patient's absence’ are for work done afterwards, without the patient.
Only one emergency allowance, and only if the criteria are met
Urgent consultation or emergency: the tariff defines each. Seeing a patient without an appointment is not, in itself, an urgent consultation.
A reason on the invoice, a diagnosis if you want one
The invoice carries the reason for treatment (illness, accident, maternity, prevention, congenital disorder). The diagnosis, however, is optional under TARDOC: the Ticino code, or just the first letter of the ICD-10-GM code. The full code is required only for outpatient flat rates.

Is Helvyca TARDOC billing software or a patient record?
Both, in a single tool. TARDOC billing is built into the patient record: the service arises from the consultation and the invoice follows from it, with no retyping and no separate software.
How much is a 20-minute consultation?
AA.00.0010 (19.20 points) plus 15 times AA.00.0020 (3.84 points), giving 76.80 points. With a tax point value of 0.91 francs, 69.89 francs. Twenty minutes is also the cap under these two positions.
What happens beyond 20 minutes?
Additional minutes are capped at 15 per session. Time beyond that can be billed only if it matches a position of its own: organ system examination, explanation of a planned procedure, patient instruction. Otherwise it cannot be billed. Helvyca points this out as you type.
Is my canton's tax point value up to date?
Each cantonal value is recorded with its source and confirmation date, then checked every January. When a canton corrects its value afterwards, Helvyca flags the invoices issued at the old value and the amount to be adjusted.
Can TARMED still be consulted?
Yes, for reference. The built-in catalogue maps the main old TARMED codes to their TARDOC successors.
Are invoices sent to insurers electronically?
Not yet. The validated invoice comes out as an archivable PDF, with a QR payment slip, ready to be sent. Electronic transmission to insurers is among the planned developments.
